Exercise Audio Files*
Many patients found it helpful to have a prompt to tell them when to
start and stop the exercises. It is difficult for some to count
seconds of holding and resting and keep track of the number of
exercises they have done. These audio files take care of that.
Individual files are available for free to patients and therapists.
They can be played right from the computer, downloaded to an MP3
player (like and I Pod), or burned to a CD. Combination files can be
purchased for download or the entire library can be purchased on CD.
The following files are in ZIP format. Please click on the link and
save it to your computer. This process might take a few minutes, then
you can click on the file and it will play on your Media Player.
* These files were created in

Stickers
If you are using paper charts you may find these stickers helpful in
recording bladder diary results or re evaluation of the pelvic floor
muscles. Just peel them off and stick them on your paper note then
write on the sticker (see example below). It saves time and space in
the chart. Must use Avery 8663 or compatible stickers. It is important to line the
stickers up correctly. Best to try a sample first.
Clinic Notes
There are many formal note templates available; some for free and some
to purchase. Here is an example of a few. All are provided in Word
2002
so you can personalize to your clinic needs. Please leave copy write
at the bottom.
Patient Education Handouts -
There are many examples of patient education handouts available.
You are welcome to use these "as is" with your company sticker added
-- leaving Beth Shelly PT credits. Or, you may use them as a jumping
point to create your own handouts.
